How to get affiliated with the NCFP

Antiquechess, a fellow chess blogger from Antique, asked on how local chess clubs can get affiliated with the National Chess Federation of the Philippines (NCFP).

I guess the question was in relation to the National Inter-Chess Club Rapid Team Championship scheduled on December 5 to 6 which allows only NCFP-affiliated chess to participate.

I hope that somebody from the NCFP read this post and provide the answer.
